Khiem Tran possesses more than 25 years of experience and holds both a Master’s Degree in Physical Therapy and a Bachelor’s Degree in Health Science from Loma Linda University. He is a dedicated caretaker whose knowledge extends to a variety of rehabilitation environments. His experience in acute care, neurology, outpatient care, hospital, and home health lead Khiem to found Physical Therapy Network Solutions with the goal to revolutionize the way that therapists and patients approach home health care, and to provide a higher quality of life for those in need.
Paula has over 30 years of PT experience, receiving her master’s degree in physical therapy from Texas Woman’s University where her career began. Paula moved to California to be near family in 2006 and expand her career through acute care, skilled nursing, neuro, outpatient ortho, aquatics, pediatrics, and home health. Paula is certified in various STM ie: Grastons and cupping techniques and is a certified personal trainer dedicated to health & fitness. When not instructing and guiding her patients /clients in restoring their physical health and well-being, she enjoys weightlifting & competitive bodybuilding.
Between dressage competitions with her horse, Encore, Amy Miller enjoys helping her patients overcome their physical challenges and reach their personal goals. Amy’s certifications as an Orthopedic Clinical Specialist and Hand Therapist, experience in the acute care hospital and rehabilitation settings provide a unique insight to the biomechanical challenges that her patients’ conditions pose. Amy received her Bachelor's Degree in Biology from Cal Poly Pomona and graduated from Western University of Health Sciences with a Master's degree in Physical Therapy. Upon her return to Cal State Fullerton University, Amy earned her Master's of Science degree in Biology.
A 1996 graduate of California State University (Long Beach), Charles McLaughlin hit the ground running by quickly advancing his position at US Healthworks to Regional Director for Southern California until his departure to found Rehab Partners, Inc. This premier home-based physical therapy staffing agency, provider, and outpatient clinic continued until 2012, when Charles shifted focus exclusively to his longtime passion of Home Health and decided to bring his talents to PTNS.
Lauren Mercado’s passion for ensuring every patient receives the best care possible is what makes Lauren such an asset at PTNS. Her past contributions to non-profit organizations providing significant therapies and resources to underprivileged families, as well as her experience in the healthcare field over the last decade, has elevated the level of care of every PTNS patient.
Dominique Aceves, UCLA graduate with a bachelors in Psychology. Extensive background volunteering in a variety of home health/healthcare organizations and with children from underprivileged backgrounds. With a passion for caring for people and years of experience in business administration and healthcare fields, Dominique plans to utilize her strengths to meet the high standards at PTNS.
With over 8 years of experience in administration, Leticia is able to tailor her skills to meet the high standards of PTNS. Leticia has proven to be indispensable in organizing ,scheduling and corresponding with clients and agencies. She is family orientated and has a passion for caring for others making her an excellent addition to PTNS.
Diane received her Physical Therapist Assistant degree from Loma Linda University and went on to Touro University Nevada to complete her doctorate degree in physical therapy. Diane has experience across various physical therapy settings including acute care, skilled nursing, and neuro. She is currently dedicated to working in outpatient orthopedics and home health care, and advocating the importance of fitness and wellness, especially among older adults and the elderly. In her downtime, she enjoys being outdoors and rock climbing.
Audrey Yonesato earned her Masters of Arts and Bachelors of Science in Occupational Therapy from the University of Southern California, and graduated with Magna and Summa Cum Laude Honors. Audrey has experience across numerous settings including hospital acute care, mental health, outpatient hands clinic, outpatient chronic pain management program, skilled nursing facilities, pediatric outpatient clinics, and home health. Audrey has attended conferences and workshops in building her specialty in oral motor feeding, and has received specialized training in providing Beckman Oral Motor Feeding Intervention. Audrey began her career providing clinic-based outpatient pediatric services for the first few years, then decided to revisit working with the adults, as she rediscovered her enjoyment and passion providing services for this population. Audrey has now been providing home health services for the past few years, as she enjoys being an instrumental part of improving patients’ abilities to overcome daily obstacles, become more autonomous and independent with important activities of daily living from an adaptive approach, and improve their quality of life in the comfort of their own home environment. During her free time, she enjoys off-roading and camping, indoor rock climbing, hot yoga, and cycling.
Richard Shum has experience treating patients of all ages and in a number of different clinical settings. With his degree in Physical Therapy and the combination of his passion and experience, Richard exceeds at increasing the level of care and excellence that PTNS sets for their staff. In his time outside of clinical practice, Richard enjoys the Southern California weather and beautiful beaches as well as riding motorcycles and enjoying retro games.
Jonathan earned his degree as a Physical Therapist Assistant from Stanbridge University and also holds a Bachelors of Science in Kinesiology from CSUF with an emphasis in Exercise Science. He found his passion for helping others through Physical Therapy and began his career in outpatient orthopedics and sports rehabilitation. Since then he has expanded his experience and has worked in a variety of settings including vestibular, aquatic therapy, acute rehabilitation, in patient hospital care and home health. Jonathan strives to provide quality care specifically tailored for each patient to help reach their individual goals. On his free time Jonathan enjoys spending time outdoors, camping, hiking, and snowboarding.
Jessica Prenovost is a graduate of Stanbridge University in Irvine, CA with Cum Laude honors and also holds a Bachelor’s of Science in Kinesiology from CSUSB with an emphasis in Allied Health Professions. Jessica started her career in physical therapy working in outpatient orthopedics where she gained a broad range of experience treating various diagnoses including post stroke and sports rehabilitation. In this setting, Jessica utilized her experience as a Pilates instructor where she had the opportunity to incorporate reformer based Pilates exercises into outpatient rehabilitation programs. She then discovered her passion for the home health setting because it allows for her to have undivided attention with each of her patients. In her free time, she enjoys surfing, rock climbing, and painting.
Lauren Souto is a graduate from Stanbridge University, with a degree as a Physical Therapist Assistant. She learned that she had a passion for helping people, especially athletes, during her time in undergrad at University of California, Irvine. She played volleyball, as walk on, for four years. Due to her athletic background, Lauren worked in an outpatient orthopedic clinic, through a unique integrative approach with a multidisciplinary team, and truly loved working with patients, doctors, nurse practitioners, acupuncturists, chiropractors, and massage therapists. She gained valuable experience in this setting and is excited to use her knowledge moving forward. In her free time, Lauren loves to be outdoors going to the beach, hiking, running, and spending time with her family, especially her nephews.
As a transplant from the East Coast, Natalia Guarino has been a licensed speech-language pathologist with a Certificate of Clinical Competency from the American Speech and Hearing Association for nearly a decade. After earning her Bachelor of Science and a Master of Arts degrees from the University of Central Florida in 2012, Natalia specialized in the fields of oral motor function (Beckman Oral Motor Assessment & Intervention) and brain injury (ACBIS 2013-2014). She also completed coursework gaining certifications for Equine Skills and Level 1 Hippotherapy from the American Hippotherapy Association; and Compton Pronouncing English as a Second Language (P-ESL) Accent Reduction.
With extensive experience in managing diverse caseloads across the lifespan (pediatric, adult and geriatric), over various facility settings (including assisted living, transitional living, outpatient clinic, inpatient acute care, skilled nursing, adult day program), Natalia enjoys collaborating with her healthcare colleagues while aiming to remain innovative as a practitioner in order to provide her speech therapy patients with the highest quality of care. When “off-the-clock”, Natalia enjoys her rewarding calling as a homemaker as well as her volunteer work as a vocalist and writer for Mariners Church.
Ashley graduated with a Master’s of Science in Occupational Therapy from Stanbridge University. She attended California State University, Northridge where she earned her Bachelors degree in Child and Adolescent Development and a minor in Asian American Studies. Prior to starting her career in occupational therapy, Ashley was an ABA therapist for children with developmental disabilities. She has experience working in settings including acute hospital, long-term acute hospital, sub-acute unit, skilled nursing facilities, schools, and pediatric outpatient clinics. She has a passion for helping others around her and giving back to her community through volunteering. In her free time she likes to explore new cities and discover new places to eat
Jasmine Truong brings a wealth of knowledge and experience to our team. She works with outpatients, acute inpatients, and home health. Jasmine worked as a certified personal trainer for 8 years and completed a certificate program for sports massage therapy in 2009 from the American Institute of Massage Therapy in Costa Mesa, CA. In 2015, she graduated from PTA school at Bay State College in Boston, MA. To further her education, she is currently working on her Bachelors in Sports and Health Sciences at American Military University.
Chanda Barrett’s wealth of knowledge and passion for personalized care is well-known among Physical Therapy Network Solutions’ clients and patients. A licensed therapist with degrees in both Physical Therapy and Kinesiology from Cerritos College and California State University, Fullerton, Chanda’s more than 10 years of expertise greatly benefits those patients with orthopedic concerns, particularly total joint replacements.
In 2016, Patricia Trinidad received her PTA degree from Gurnick Academy of Medical Arts in San Mateo, California. Her background includes inpatient physical therapy and outpatient physical therapy. One of the aspects Patricia loves most about being a Physical Therapist Assistant is meeting new people and joining them on their journey to wellness and fitness. Her desire is to assist patients in achieving their optimal health potential, fitness, and functional goals. To achieve this, she provides education to enable understanding of how the body moves and heals. Her philosophy is to teach and train each person so they are given the tools needed to maintain their health.
Born and raised in Southern California, Maria Plasencia graduated from California State University, Fullerton with a bachelor's degree in Kinesiology and an emphasis in clinical exercise. Maria’s passion for Physical Therapy during personal rehabilitation for an injury sustained while attending college and her patients’ healing journeys continue to inspire her focus on providing the highest quality of care. Her additional training from Stanbridge University provided a comprehensive understanding in both outpatient orthopedics and home therapy for her current position as a dedicated Physical Therapist Assistant.
Gabrielle Lovato is a graduate of Stanbridge University in Irvine, CA with Cum Laude honors and also holds a Bachelor’s of Science in Kinesiology from CSULA with an emphasis in Rehabilitation and Therapeutic Sciences. Her experiences in an outpatient orthopedic setting providing care for patients suffering from lymphedema, TMJ, post-surgical repairs, gait dysfunctions (etc.) extended her hands-on knowledge base with numerous manual techniques such as myofascial release, IASTM techniques and MDL (manual lymphatic drainage). She also has previous experience in inpatient neurology where she was able to work with patients suffering from TBI, stroke and total joint replacements. Gabrielle recently transitioned into home health physical therapy in hopes of making a positive, fun and creative therapeutic environment for every patient that she works with. She believes that patients should not all be given the same therapy plan, but are all different from one another and need an individualized plan to allow patients to reach their maximum potential.
Jazmin developed a passion for movement and exercise growing up as a competitive gymnast. She took her love of exercise to San Diego State University where she graduated with a Bachelor’s of Science in Kinesiology. After obtaining her B.S. degree, Jazmin went on to receive her Doctorate of Physical Therapy (DPT) from Azusa Pacific University. Throughout her time at APU, she gained experience in orthopedics and inpatient rehabilitation where she worked with a variety of patients including, stroke and TBI. After her time at APU, she went on to focus her physical therapy career in orthopedics.
In her spare time, Jazmin enjoys camping, spending time with her dog, and snowboarding. She is also a home baker and world traveler, always looking for the next baking challenge or destination to explore.
Haily received her Physical Therapist Assistant degree from Concorde Career College and also holds a Bachelors in Communications and a minor in Psychology from Providence Christian College. She has experience in outpatient and inpatient orthopedic settings helping patients implement stretch and exercise programs designed to improve patients' level of mobility, strength and cardiovascular conditioning as well as improve overall quality of life. She also has experience in Pediatrics working to held guide the children and their families through comprehensive therapy programs to address functional deficits and needs based on age and developmental levels. She is passionate about helping each patient reach their goals and maximum potential through one on one sessions that help boost their confidence and become more independent in their daily life. In her free time she enjoys taking her dogs to the beach, hiking, and working out.
Ernesto is a graduate of Stanbridge University where he received his degree to be a Physical Therapist Assistant. He has experience in a variety of healthcare settings such as skilled nursing facilities and home based therapy. Additionally, Ernesto is fluent in both Spanish and English. In his free time he enjoys hiking and going to the beach.
Sarah Little received her Physical Therapist Assistant certificate from Stanbridge University in Irvine, Ca with Cum Laude honors and also holds a Bachelors degree in Science, Kinesiology from Cal State Fullerton. Most of her experience is in the pediatric setting helping guide children and their families through therapeutic programs that help improve gross motor delays and overall functional movement. She has a passion for helping others achieve their goals and improve their quality of life through one on one sessions. In her free time, she enjoys going on hikes, exploring the outdoors, running and spending time with her family.
Tina earned her Master of Arts degree in Occupational Therapy from USC and Bachelor of Arts degrees in Art History and Sociology from UCLA. She brings to PTNS extensive experience in geriatrics and physical disabilities, having worked for the past 14 years at a skilled nursing facility. She is delighted to once again be using her skills towards helping others to live more fulfilled, independent lives; prior to becoming an occupational therapist, she worked as an independent living skills instructor for adults with developmental disabilities. In her spare time, Tina enjoys baking, yoga, eating tacos, reading nonfiction, thrift shopping and spending time with her senior dog Jasper.
